about your medications & treatments.

for home care services, Public Health will bill Medicare, Medicaid, or your
private insurance. Additional funding sources may be available.

Skilled Nursing Services

Evaluate
client's nursing needs.

Initiate
the Physician ordered Plan of Care and necessary revisions.

Provide
skilled nursing services as ordered:
injections, blood draws, wound
care, diabetic teaching, disease
management, etc.

Coordinate
all services required by the client.

Participate
in Case Management for the Frail and Elderly Waiver Program.
·
Assist
frail and elderly in long-term planning to stay in home.

Client
and family health teaching.

Provide
for DHS/court ordered child or adult protection services.
